We are looking for 1 nurse to work at the day hospital specializing in geriatrics and nutrition. This position is 50% (7 hours Monday / 7 hours Tuesday / 3:30 hours Wednesday). In order to provide geriatric workshops (two workshops), training in therapeutic education would be appreciated. Hours: Monday and Tuesday 9am-5pm with 1 hour break / Wednesday 1:30pm-5pm - Receiving pre-admission requests and making appointments for pre-admission consultations on dedicated days - After consultation and validation of the pre-admission with the doctor: - Conducts an interview with the patient, - Gives him the welcome booklet and his workshop program, - Coordinates the increment of appointments and series of appointments for paramedics in the diaries - Enters new patients into the HM care software (Pre-admission then admission), completes the file with the trusted person, the person to be notified, codes the dependency, codes his actions, assesses the risk of falling, assesses the pain and carries out his transmissions. - Integrates patient documents into HM - Welcomes patients and directs them to workshops - Organizes workshop planning in conjunction with the coordinator and paramedics by scheduling initial, intermediate and final assessments - Conducts educational workshops on Monday mornings (from 10 a.m. to 10:45 a.m.) and Tuesday mornings (from 10 a.m. to 10:45 a.m.), carries out coding and transmissions relating to these workshops. - Participation in multi-professional staff - Manages the equipment in his treatment room, checks the expiry dates of medical devices monthly, reports monitoring materials to the health manager in charge of this part.
