We are seeking a full-time or part-time General Practitioner/Geriatrician (M/F) for our partner client, on a permanent contract. You will work in two of their nursing homes (EHPADs) (63 beds and 35 beds) located in the Aude department, in the Corbières region, 40 minutes from Narbonne and 45 minutes from Perpignan, ideally situated near both the sea and the mountains! You will be part of a multidisciplinary team within the Aude region's elderly care pathway, supporting dependent elderly individuals to maintain their autonomy, family and social connections, and prevent complications related to their vulnerability. You will be responsible for developing and monitoring the facility's care plan, conducting medical assessments of residents using your gerontological expertise, and leading the care team. Regarding the resident's gerontological assessment, you will perform various evaluations (cognitive tests, nutritional assessments, etc.), the results of which will allow the care team to plan interventions for the resident. You will coordinate the interventions of various professionals, liaise with external providers (physiotherapists, speech therapists, etc.) as well as general practitioners and specialists. Together with the nursing home's coordinating nurse, you will ensure the proper maintenance of medical and paramedical records. You will participate in the implementation of agreements with healthcare facilities, gerontological networks, and other stakeholders in the geriatric care pathway. You may also be involved in the Corbières Territorial Resource Center project.
