We are seeking a full-time or part-time General Practitioner/Geriatrician (M/F) for our partner client, on a permanent contract. You will work in two of their nursing homes (EHPADs) (63 beds and 35 beds). Located in the Aude department, in the Corbières region, the facility is 40 minutes from Narbonne and 45 minutes from Perpignan, close to both the sea and the mountains. You will be part of a multidisciplinary team within the Aude region's elderly care pathway, supporting dependent elderly individuals to maintain their autonomy, family and social connections, and prevent complications related to their vulnerability. Your responsibilities will include developing and monitoring the facility's care plan, conducting medical assessments of residents using your gerontological expertise, and leading the care team. Regarding the resident's geriatric assessment, you will conduct several evaluations (cognitive tests, nutritional assessment, etc.) whose results allow the care team to organize interventions for the resident. You will coordinate the interventions of the various professionals involved, and you will liaise with external providers (physiotherapist, speech therapist, etc.) as well as with general practitioners and specialists. Together with the nursing home's coordinating nurse, you will ensure the proper maintenance of medical and paramedical records. You will participate in the implementation of agreements with healthcare facilities and geriatric networks, or any other stakeholders in the geriatric care pathway. You may also participate in the Corbières Territorial Resource Center project. You will work with the medical and paramedical team of the elderly care pathways and Medical and Rehabilitation Care (SMR) services for the Aude region of the partner association. - Permanent part-time or full-time contract - Salary according to national collective agreement 51 FEHAP, Ségur bonus, numerous social benefits... a job description is available upon request
